If requested, I will provide access to my property, for a representative of HL&P, to verify installation. I will also provide charger utilization data upon request.

I understand that a rebate will not be paid if I refuse any of the above requests. By signing this form, I certify that I have purchased and installed the EV Charger for use at the address noted above. I have read and understand the terms and conditions of the EV Charger Rebate program.

In addition, the installation of the charger complies with applicable permitting requirements and the contractor used to perform the work holds the appropriate license.

Terms and Conditions:

• Applicant must be a HL&P customer with an active account. Electric Vehicle must be registered with the UT DMV at the address of an active HL&P account.
• This application must be signed, dated, and contain ALL supporting documentation.
• HL&P is not responsible for items lost or destroyed in the mail. Rebates are not guaranteed, and some limitations apply.
• Application must be submitted no longer than 90 days from the date of charger purchase. Rebate applications are accepted on a first-come, first-serve basis while funds are available. A rebate check will be mailed within 6-8 weeks of HL&P receiving the application.
• Rebates are limited to one (1) rebate per residential account. If applicable, the applicant must have approval from the property manager, owner, or HOA to install the charger.
• Charger(s) must be Level 2 (240V) and utilize the SAE J1772 charging plug or Tesla’s High Power Wall Connector and be UL or equivalent listed. Chargers with greater capacity will be considered on a case-by-case basis.
• Charger must be a new purchase with installation hardwired to the property’s electrical service.
• The EV must be DOT approved for highway application. Golf carts, neighborhood carts, motorcycles, electric scooters or bicycles, and other low-speed vehicles are not eligible.
• Installation of the charger must comply with permitting requirements and the installing contractor must hold the appropriate license.
